Standard Certification Request Form

Note: this is for Standard Recommendation only

Student Affidavit:
I have completed the following:

Phase 1

• Completed all coursework and student teaching or internship
• Met curriculum requirements with a 2.50 GPA and grades of C or higher in all coursework
• Passed state-mandated tests
• Paid all financial obligations to the university

Phase 2

• Applied for standard recommendation at the TEA website
  • Go to www.tea.state.tx.us
  • Log in to TEA website
  • Choose APPLICATIONS
  • Choose STANDARD CERTIFICATE TEXAS PROGRAM
  • Follow the prompts, which will require paying a credentialing fee and a fingerprinting fee
• Updated my contact information (personal--not LETU--email, mailing address and phone number) at www.tea.state.tx.us
• Waited for:
  • The Registrar to post all of my grades
  • TEA to post passing scores for state-mandated tests

Further,  I understand that if I have not completed Phases 1 & 2, this request for recommendation will be rejected and I will need to make another request when Phases 1 & 2 have been completed
